Employee Funded Leave Program (EFLP)
Employee Funded Leave Program
Directive - Full time Employees with a minimum of 12 months continuous service, can apply to finance a leave of absence for any purpose by deferring part of their salary for a minimum of 12 months to a maximum of 6 years. The Employee must apply and be approved by their Ministry's Deputy Head in order to participate.
Form - Program Application 
Application to enroll in the Employee Funded Leave Program. The completed applications must be forwarded to the Employee's Human Resources Office within their Ministry.
Form - Change Request 
Complete the Change Request form if circumstances arise that require the Ministry or the Employee to make a change to the original information that was submitted on the Employee Funded Leave Program application form. The completed Change Request form must be forwarded to the Employee's Human Resources Office within their Ministry.
Form - Program Cancellation 
Complete the Program Cancellation form if an Employee needs to withdraw from the Employee Funded Leave Program. The completed Program Cancellation form must be forwarded to the Employee's Human Resources Office within their Ministry.
Form - Benefit Payment Authorization & Direction 
Complete the Benefit Payment Authorization and Direction for approximately 3 months prior to the commencement of the leave. The completed form must be forwarded to the Employee's Human Resources Office within their Ministry.
